Health & Wellbeing Coaching

Health and wellbeing coaching is a person-centred approach. The coach works alongside the resident to build confidence, overcome barriers and develop skills to help manage their health needs. The program starts with an initial assessment to help identify support needed and set health goals.

What is this service?

About the Service

Health and wellbeing coaching is a person‑centred service that supports residents to understand what matters to them, build confidence, and make positive long‑term changes related to their long term health conditions. Coaches work alongside residents to explore goals, identify barriers, and develop the skills needed to manage their own health.

Eligibility

You may be referred to the LTC+3 Coaching Service if you have:

  • Three or more long‑term conditions

  • Two confirmed long‑term conditions and one suspected

  • One confirmed long‑term condition and two suspected

Eligible long‑term conditions: Hypertension, Diabetes, Atrial Fibrillation, Chronic Kidney Disease

Who We Cannot Support

We are unable to accept referrals for residents who are:

  • Under 18

  • Living with moderate to severe frailty

  • Medically optimised (to have the conditions but are managing them well)

  • In mental health crisis

How the Programme Works

  • Initial assessment: We discuss health, lifestyle and priorities.

    Goal setting: We agree realistic goals and identify the support needed.

  • Coaching sessions: Held every two weeks, usually 6-8 sessions, to maintain motivation and review progress.

  • Follow‑up: A check‑in 3–6 months after completing the programme to see how progress is being maintained.
